Meet Samantha: The Intelligent, Automated, Real-Time EHR Virtual Assistant & Transcriptionist

Minimizing the time required to complete the charting process allows physicians to spend more time with patients and helps alleviate frustration & burnout

Boston, Mass. – Oct. 3, 2017 – Meet Samantha, a virtual assistant that acts as an intelligent medical transcriptionist or scribe for healthcare providers, from NoteSwift, Inc.

Samantha’s patent-pending Parsing and Dynamic Matching (PDM) technology takes the physician’s natural language input of a patient encounter and automatically identifies the discrete data elements versus the narrative text, assigns the required coding, and puts its all into the EHR exactly where it needs to be, just as if the provider had manually entered it themselves.

Samantha integrates seamlessly with the EHR (currently available for Allscripts Professional, with more EHRs launching later this year) and operates from a single screen, thereby reducing the burden on the physician of having to navigate the many EHR menus screens, check boxes and coding requirements. As a result, physicians can document patient notes in at least half the time.

“Samantha is a true game-changer and will disrupt the EHR charting process,” says Dr. Chris Russell, a neurologist at Peachtree Neurological Clinic and Piedmont Healthcare in Atlanta, Georgia, and Chief Medical Officer at NoteSwift. “I’m able to complete my patient notes much faster and with greater accuracy. The fact that I don’t have to jump from screen to screen and click 100 times has reduced the anxiety that used to accompany patient note entry.”

Samantha vastly improves the EHR documentation process in Allscripts Professional EHR when compared to existing workflows by:

  • Entering documentation faster and saving physicians 6-8 hours on average each week, giving them more time to see patients and to follow up on co-morbidities for better patient care.
  • Automatically adding billing codes, accurately and intuitively. Claims are submitted faster, while reimbursement denials, delays and appeals are dramatically reduced.
  • Eliminating the costs and delays of scribes, coders, and transcriptionists.
  • Automatically entering information as structured, discrete data, so it can be searched and reported on in support of Population Health initiatives, ultimately resulting in better patient care over the long term.

Through her natural language understanding and EHR integration, Samantha knows that when, for example, a physician says, “ECG 12 leads,” the EHR is looking for “tracing only of routine 12 lead electrocardiograms (CPT code 93005),” and applies the complete details to the note. The results are entered as structured, discrete data and narrative text with the corresponding coding assigned automatically.

Samantha matches dictated terms with the proper equivalent in the EHR, and detects potential conflicts or ambiguous terms, automatically prompting the user to review and correct any potential conflicts, ensuring maximum accuracy. Samantha also learns from the provider and will improve upon the tasks that the physician most commonly performs over time—just like a transcriptionist, but in real time.

“The big challenge in the market today is that each EHR has its own unique way of documenting the patient note,” explains NoteSwift, Inc., President and CEO Wayne Crandall. “Any viable solution must have the ability to be seamlessly integrated with each supported EHR in order to ensure that the structured data output is placed correctly, and we designed Samantha to do just that.”

Samantha is available with or without built-in speech recognition, and supports most popular medical speech recognition products, including Nuance Dragon® Medical One, Dragon® Medical Practice Edition, and Dragon® Medical Network Edition, as well as M*Modal Fluency DirectTM and nVoq SayItTM. Samantha can also be used without dictation, and works with the EHR in local, Cloud- or on premise-hosted-RDP or Citrix environments. Prices range from $49-$99 per month, per provider.

NoteSwift, Inc., Brings Machine Learning and Artificial Intelligence to EHR Charting

Patent-pending, proprietary PDM technology saves physician users 6-8 hours a week

Boston, Mass. – September 27, 2017 – NoteSwift, Inc., has set out to disrupt the EHR charting process by incorporating components of machine learning and artificial intelligence to develop a tailored, heuristic approach to solving the challenges that plague the EHR patient note input process.

NoteSwift’s patent-pending Parsing and Dynamic Matching (PDM) technology allows physicians to dictate or type a narrative of the patient encounter without having to wade through layers of menu screens, navigate hundreds of mouse clicks and check boxes, or spend time looking up ICD10, SNOMED, and CPT codes. As a result, physicians have more time to treat patients, document co-morbidities, and will also see a reduction in claim denials and resubmissions, thanks to improved reporting accuracy.

“Speech recognition is often cited as a way to get narrative information into the EHR, but that is only part of the solution;” explains NoteSwift VP of Research and Development Allan Stratton. “With our new approach, we take the recognized text from any speech recognition product and apply our proprietary PDM technology to provide a complete solution that specifically addresses and alleviates the challenges associated with patient charting.”

NoteSwift’s PDM technology takes narrative input and parses and dynamically matches the data to discern the key elements of the stated phrases within the note. For example, “ECG 12 leads” is identified within the note and the PDM understands that the EHR is looking for “tracing only of routine 12 lead electrocardiogram (CPT code 93005)” and automatically applies it. The results are entered as structured, discrete data and narrative text with the corresponding coding assigned. The data then is entered seamlessly into the EHR as if the check boxes, hundreds of mouse clicks, and other manual entry tasks were performed by the user.

“The promise of NLP and machine learning technologies are advancing, but have yet to be applicable to the challenge of entering patient notes — until now,” says NoteSwift President and CEO Wayne Crandall. “Our team has developed this PDM technology and packaged it into a user-friendly, cost-effective product that solves the unique challenges of EHR patient charting, and ultimately helps reduce physician burnout, which several recent studies have shown to be on the rise as a result of EHR use.” [1],[2]

NoteSwift’s PDM technology is the foundation of the company’s newest product offering, Samantha, and can be used with anything that produces narrative text and supports the major speech recognition products, including Nuance Dragon® Medical One, Dragon® Medical Practice Edition, and Dragon® Medical Network Edition, as well as M*Modal Fluency DirectTM and nVoq SayItTM.

NoteSwift CEO Wayne Crandall spoke with Healthcare Tech Outlook about Samantha, and our continued commitment to developing innovative ways to improve the EHR user experience for medical providers.

Read on for an excerpt from the article:

“Not only are providers spending too much time on patient notes, healthcare facilities often also employ staff to recheck those notes to ensure accuracy, resulting in even higher costs, lower revenues, and less efficiency,” says NoteSwift CEO Wayne Crandall.

Thanks to NoteSwift’s patent-pending semantic understanding and advanced parsing technology, Samantha is a game-changer in the EHR industry. With its own innate medical speech recognition system, natural language understanding, machine learning capabilities, and data structure analytics, Samantha captures patient notes in real-time and feeds the information accurately into the EHR as structured and coded data.

“Physicians simply dictate or type the complete narrative of patient interactions and the rest is done automatically within seconds,” Crandall explains. The solution ensures that the correct terminology, words, and data formats are used to avoid errors and claim denials. Hospitals and healthcare providers can now submit reimbursement claims faster with fewer insurance denials, as Samantha also assigns all the medical coding with precision—without the aid of any transcriptionists or medical staff.

Samantha by NoteSwift wins Product Evolution Award at the 2017 Allscripts Developer Summit

NoteSwift’s revolutionary new product, Samantha — your real-time, automated EHR transcriptionist — was selected as winner of the Product Evolution Award at the 2017 Allscripts Developer Summit in San Francisco, CA.

Samantha is the latest revolutionary product from NoteSwift. With her own built-in speech recognition capabilities and patent-pending technology, Samantha intelligently parses dictated or typed patient note information and automatically places it as structured, discrete data and narrative text, directly into Allscripts Professional EHR™.

“Innovative companies are always improving their functionality, re-imagining their products and taking advantage of the latest technology available,” explains Tina Joros, VP & General Manager of Allscripts’ Open Business Unit. “The Samantha product by NoteSwift encompasses all these qualities and we are proud to award them with our Product Evolution Award for 2017.”

Because Samantha is seamlessly integrated with Allscripts Professional EHR, she understands the specific rules for each section of the note and applies all necessary coding, including ICD-10, SNOMED, and CPT codes. Her semantic intelligence enables her to match dictated terms with the proper equivalent in the EHR, and to detect potential conflicts or ambiguous terms, and enable them to be corrected easily before the note is signed-off on and finalized.

Samantha’s proprietary intelligent technology also learns from the provider and will perform better over time on the tasks that the physician most commonly performs — just like a professional transcriptionist, but in real time.

“Samantha is truly a game changer,” says Dr. Chris Russell, a practicing neurologist and NoteSwift’s Founder and Chief Medical Officer. “She dramatically speeds up the patient note entry process and saves me and my team an average of 6 hours each week in EHR documentation time.”

NoteSwift Named ‘Most Innovative’ Solution at Allscripts 2016 Developer Summit

Second Award Presented for Strategic Influence, Allscripts Reselling NoteSwift to Small Practices as Preferred Add-on

Boston, Mass. June, 8 2016 – NoteSwift, Inc. received two awards at the Allscripts Developer Summit in Chicago: Most Innovative Solution and Strategic Influence.

“These awards affirm the joint efforts of NoteSwift and Allscripts to accelerate and enhance physician workflow. We look forward to expanding existing products and creating new initiatives in our strategic relationship with Allscripts with an emphasis on making the EHR user experience as simple as possible,” said Wayne Crandall, President and CEO at NoteSwift.

NoteSwift enables healthcare providers using Allscripts® EHRs with Dragon® Medical speech recognition to enter patient notes 40% faster than with the EHR alone. With NoteSwift, providers use their voice to navigate through the EHR, and NoteSwift accurately places both structured data and narrative text into the patient note. Mouse clicks and keystrokes are reduced from more than 100 per note to less than five. With the time saved, NoteSwift clients report that they see more patients and get home earlier.

“NoteSwift has transformed my EMR experience. The usual 1.5 to 2 hours of charting after each day has been reduced so that I generally leave the office with all charting complete about 30 minutes after the last patient. My family has me back. The computer is working for me rather than the other way around,” said Pediatrician Dr. Eliot Hall of Rockingham Medical Group, Bellows Falls, Vt.

“What NoteSwift has done in simplifying yet speeding up clinical documentation is remarkable. Combining our open APIs and voice recognition software is exactly the kind of hybrid solution we dream about, combining technologies to build solutions that will delight our common clients,” said Stanley Crane, Chief Innovation Officer at Allscripts.

NoteSwift 3.0 Makes Entering Patient Notes in Allscripts Professional EHR™ 40% Faster

Entire patient notes entered with fewer than 5 mouse clicks

Boston, Mass. January 11, 2016 – NoteSwift, Inc. has announced the release of the 3.0 version of NoteSwift, which enables healthcare providers using Allscripts Professional EHR with Dragon Medical speech recognition to enter patient notes 40% faster than with Allscripts Professional alone. With NoteSwift, providers use just their voice to place both structured data and narrative text into the patient note, reducing over 100 annoying mouse clicks and keystrokes to less than five. In addition, NoteSwift links the narrative details with the structured data, enhancing the overall clarity and completeness of the chart.

This latest version of NoteSwift includes robust new time-saving and frustration-reducing features for the most intensively used sections of the patient note:

  • Reason for Visit – Just say the symptom and NoteSwift automatically searches for it. Multiple findings can be entered in a single voice stream without the need to repeat “Next finding.” Entries are made via natural dictation, so that “Week: 2” becomes “2 weeks ago.”
  • Assessment & Plan – When updating current medications, simply speak the patient’s past and current medications into the note instead of time-consuming manual keyboard entry.
  • Prescribe – NoteSwift’s new Med-Alias file immediately recognizes the top 100 prescribed drugs. Simply say the drug name, strength, dosage, days, quantity and refills. In less than 5 seconds the e-prescription is ready to be sent to the patient’s local or mail-order pharmacy.
  • History – Meet MU requirements by automatically entering Family and Social History as structured data.

Other enhancements include context-sensitive help based on each section of the note, and the ability to use a single profile for local and remote access.

“Physicians are complaining loudly about the time they spend, both in the office and at home, on electronic health records. NoteSwift users tell us that by reducing the time they spend on patient documentation, they can see at least two more patients per day and get home earlier,” said Wayne Crandall, President and CEO of NoteSwift.

Users of NoteSwift 2.0 with a current maintenance contract will be contacted by NoteSwift regarding a complimentary upgrade to the 3.0 version.
